Affirm is primarily funded by the U.S. Department of Health and Human Services through the Title X (“ten”) program. Affirm receives additional funding from donors, community foundations and state organizations for specific projects or to broaden the scope of services.
Most of our clients cannot afford medical care and/or do not have health insurance. Our aim is to serve people in Arizona – especially youth, the underinsured and uninsured, people who earn low wages, Indigenous communities, and other groups who have been systematically excluded from receiving care.

Congress established Title X (“ten”) legislation with broad bipartisan support, and it was signed into law by President Nixon in 1970. Title X is the only dedicated source of federal funding for family planning/ sexual and reproductive health care services. Health centers receiving Title X funds are required to provide reproductive health services and related preventive health services, prioritizing clients that receive low wages or are uninsured. You don’t need insurance to be seen at a Title X health center. If you have insurance, it may pay for your visit. Bring your insurance card (AHCCCS and/or private) with you to your appointment.
Our clients may be tested for sexually transmitted infections/diseases to prevent infertility caused by certain STI/STDs. Getting tested regularly for STI/STDs is key to overall health. Treatment for certain sexually transmitted infections (chlamydia, gonorrhea, syphilis, etc.) is covered by Title X. For others like HIV, you will receive a referral for treatment.
Health centers are staffed by medical doctors, physician assistants, nurse practitioners, registered nurses and medical assistants that provide high-quality healthcare.

During your visit, you will fill out forms with information on your health and income. You are not required to fill out any information you do not wish to report. A medical assistant will review your medical history and may take your vital signs and/or take urine and blood samples for testing. From there, the services depend on your current health and needs. You may have a physical exam. You may receive counseling and education. You may receive a form of birth control. Some patients may receive referrals for other services or treatment.

Title X services are free ($0) for those whose household incomes are below 100% of the Federal Poverty Level. Health centers take into account your income and how many people are in your household. Otherwise, costs are discounted, based on a sliding fee scale, for clients with household incomes up to 250% of the poverty level. Teens who go to appointments without a parent/guardian present will pay (or not) based on their income level. Call a nearby Title X health center to find out what you will pay.
Yes! Services for those assigned male at birth include (but may not be limited to) testing & treatment of sexually transmitted infections (STIs), education on the prevention of STIs & unwanted pregnancies and reproductive life counseling. For your physical exam, the clinician may examine your penis and testicles to check for any lumps, bumps or sores. They may also check for hernias.

Everything you share during your visit is confidential, except in the case of disclosures that must be reported to the proper authorities. These include: sexual activity under the age of 14, incest, child molestation, abuse, human trafficking, rape or intimate partner violence. Positive test results for certain sexually transmitted infections may be reported to the state health department.
It is important for the health center to have a way to reach you about test results, however, staff will not leave a message or share your information with anyone else unless you give them permission to do so.
Pregnancy options counseling gives information about the options available after someone determines they are pregnant- usually confirmed by a pregnancy test. Information is non-directive, meaning it is based on what the pregnant person wants and not what the provider thinks is best. The information includes how to access services that help during the pregnancy and after childbirth. Options counseling may include information about healthy pregnancy, ending the pregnancy and adoption. Title X does not cover prenatal care, abortion or adoption services.
Anyone who receives abnormal results from a Pap smear or breast/chest exam will receive referrals for further evaluation and/or treatment.
Yes. People 17 and younger are eligible to receive all of our services.
Minors are not required to bring a parent or guardian when receiving care. Affirm encourages teens to speak openly with their parents/guardians about any activities that could potentially affect their health, including their sexual activities, however, they do not need to consent or be present for a minor to access Title X care.
